Health Physics Society Membership Application

To apply for Health Physics Society membership, use the form below and:

  1. Complete your name, address, phone, fax, and email information.
     
  2. Select a membership category. Plenary membership requires a bachelor's degree, associate's degree, NRRPT registration, and several years professional experience. If you apply for plenary membership, you will receive associate membership now, and a full application will be mailed to you. Associate membership has no degree or membership requirements.
     
  3. If you would like to join a section, select as many as you like ($5.00 fee per selection) by clicking on the appropriate section(s). Use the CTRL and SHIFT keys to select multiple sections.
     
  4. Click on the CONTINUE button, and follow the instructions on the next page.
